Slow shutter speed chosen in low light
A photograph that looks soft all over, subject and background alike, usually points to one cause: the shutter was open too long for a handheld camera to hold still. Every hand has some tremor, and that tremor smears the image across the sensor the moment shutter speed drops below a safe threshold — a photographer working indoors at dusk, or in a dim room without flash, is the classic setup for this failure.
The commonly cited handheld safety rule ties shutter speed to focal length: use a shutter speed no slower than one over the focal length in use, so a 50mm lens wants at least 1/50s and a 200mm lens wants at least 1/200s, doubled or more if the camera or lens has no stabilization. When the whole frame is soft rather than just the subject, this is the first thing to check, because the fix is one of three moves and only one of them fits a given scene:
- Raise the shutter speed and accept a darker frame, then compensate elsewhere
- Raise the ISO so the sensor needs less light to reach the same brightness at a faster shutter speed
- Put the camera on a tripod or brace it against something solid, which removes the handheld constraint entirely and lets shutter speed drop as low as the scene requires
Choosing between these three is really about what's left to sacrifice — a moving subject rules out the slow-shutter-plus-tripod option even if the tripod is available, because the tripod stops camera shake but does nothing for a subject that's moving.
Wide aperture set on a suitable lens
A background that dissolves into soft shapes behind a subject is not an accident of the lens — it's aperture doing its job. Opening the aperture wide (a low f-number, such as f/1.8 or f/2.8) narrows the zone of the picture that's in focus, called depth of field, so anything not on the same plane as the subject falls progressively out of focus the farther it sits from that plane.
The trade-off is that the "in-focus zone" gets thin enough that missing it by a few centimeters ruins the frame. In a portrait shot at a wide aperture, that zone is often only a few centimeters deep, so focus has to land precisely on the subject — for a person, that almost always means the eyes rather than the nose or ear, since a viewer's attention goes to the eyes first and any softness there reads as a failed photo even if the rest of the frame is technically fine.
Two things solve this at once: use single-point autofocus rather than a wide focus area so the camera doesn't guess which part of the scene matters, and choose the sharpest point on the subject as that single point before the frame is taken. Narrowing the aperture a stop or two, from f/1.8 to f/2.8, for example, buys a slightly deeper zone without giving up the separated-background look.
Shooting under indoor or mixed artificial light
An orange or blue cast across a photograph is a white balance failure, not an exposure failure — the picture can be perfectly bright and still look wrong in color. Different light sources emit different color temperatures, and a camera's automatic white balance is guessing at that temperature from the scene in front of it; under mixed lighting (a window plus a lamp, or fluorescent plus flash) that guess often lands wrong, tinting the whole frame.
Two ways around it:

That second row is the reason raw capture matters more for color problems than for most other exposure mistakes: a raw file preserves the original sensor data, so white balance can be reset after the fact as if it were never applied, while a JPEG has already committed to one interpretation of the light.
Camera left in a fully automatic mode
A subject that comes out too dark against a bright background, or too bright against a dark one, is usually a metering problem, not a lighting problem — the camera exposed for the average of the whole frame rather than for the person or object that mattered. Fully automatic mode makes that averaging decision without knowing what the subject is, which is fine for an evenly lit scene and wrong the moment the frame has strong contrast: a person standing in front of a bright window, a subject against snow, a face in a shadowed doorway.
Moving out of full auto doesn't mean giving up help entirely. Aperture-priority mode keeps the camera choosing shutter speed while the photographer sets aperture and can dial in exposure compensation to override the average the meter picked — pushing the exposure up for a subject the camera read as too bright, or down for one it read as too dark. Manual mode goes further, setting aperture, shutter speed and ISO independently, which matters once a scene has both a moving subject and tricky lighting, since no single automatic mode can solve both at once. The ordering that makes manual mode workable: decide which control the scene most needs fixed first (motion blur means shutter speed goes first, depth of field means aperture goes first), set that one, then adjust ISO to bring the meter reading back to the target for that subject.
Exposure
Exposure is the overall brightness of a photograph, and it's never set by one control alone — aperture, shutter speed and ISO all contribute, and each one also changes something else about the image besides brightness. Aperture affects depth of field, shutter speed affects motion blur, and ISO affects how much visible noise or grain appears in the final file, with noise increasing as ISO climbs. The four fixes above are really one fix seen from four different symptoms: soft frame, blurred background, wrong color, wrong brightness for the subject. Reading which symptom is on screen, and tracing it back to the one setting that produced it, is the skill; the next photograph taken with that diagnosis in mind is where it gets tested.
